How It Works
A care team that works alongside your doctor
Every Story Health program is built around three things: a health coach who knows you, AI-powered monitoring that catches changes between visits, and a direct line back to your regular doctor.
A health coach in your corner: Regular check-ins, medication support, and someone to call without having to wait for your next appointment.
Monitoring from home: We send you a connected device - like a blood pressure cuff – at no cost. Your readings go directly to your care team.
Clinical oversight from NPs: A Story Health nurse practitioner reviews your care plan, can adjust medications, and escalates to your doctor when needed.
Your doctor sees everything: We share structured updates with your healthcare provider throughout your program. They see every change to your care plan.
What to Expect
From your first day to your last check-in.
Meet your health coach: Within a few days of enrolling, your coach introduces themselves, reviews your care plan, and gets you set up.
Clinical visit: A Story Health nurse practitioner establishes your baseline, reconciles your medications, and activates your care plan.
Ongoing monitoring between visits: AI tracks your readings continuously, your coach checks in regularly, and a clinician steps in when needed.
Regular updates to your doctor: Structured clinical summaries go back to your doctor throughout your program.
Enrolled in CMS ACCESS? Your care is covered by Medicare.
If your doctor referred you to a Story Health program through CMS ACCESS, your care is completely free. No copay, no bill, no change to your existing Medicare coverage.

Common Questions
Do I have to participate?
No. Every Story Health program is voluntary. Your doctor referred you because they believe it would help — the decision is yours.
Will I still see my regular doctor?
Yes. Story Health supports your care between visits, not instead of your regular team. All your appointments, specialist referrals, labs, and imaging stay exactly as they are.
What does it cost?
For patients enrolled through CMS ACCESS, there is no cost. No copay, no bill from Story Health, ever. For other programs, your care coordinator will explain any costs at enrollment.
What information is shared about me?
We send regular clinical updates — your measures, medications, and care plan progress — to your doctor. All sharing requires your consent and complies with HIPAA.
Can I leave the program?
Yes, at any time. Contact your health coach or call the number on your enrollment materials.
